Family+ Healthcare Membership Charleston: Questions

Who can join, how the price works, and what happens when life changes.

Family+ starts from $135 a month per household for one adult and one child. Two adults with no children are $145 a month. The minimum household is two people.
It is built per person. The main adult is $85 a month, a dependent adult is $60 a month and each child is $50 a month. Moving up a level adds $10 per person per month.
Yes, from six months old. We don’t take babies under six months.
Yes. A partner at the same address is an extra adult — $60 a month at Essential.
Yes. Two adults at one address are a Family+ household: $85 for the first adult plus $60 for the second, $145 a month at Essential. Personal+ covers one adult only.
Yes. A son or daughter over 18 who lives at your address can be on Family+ as an extra adult.
Yes. A parent or grandparent living at your address can join as an extra adult.
Essential covers in-clinic primary care and urgent care. Enhanced adds 24/7 Virtual Urgent Care+ and the RX+ Pharmacy Program. Complete adds Therapy+, 12 sessions with a licensed therapist per major life event, on top of everything in Enhanced. Each step up is $10 more per person per month.
Insurance is for the big things. For everyday care it usually means a deductible first and a bill after the visit — often weeks later. With Family+, your primary and urgent care is $0 and you know the cost before you walk in. Keep your insurance for hospital, ER, surgery and specialists.
No. No insurance is needed. Family+ is a direct membership with Holy City Med.
No. Family+ is not insurance. It doesn’t cover hospital stays, the ER, surgery or specialists, so we still recommend a plan for the big things.
Call 911 or go to the nearest emergency room. Emergency care sits outside the membership. For everything else — fevers, earaches, sprains — we’re here, same day.
RX+ is included at the Enhanced and Complete levels: common urgent-care medications at 70,000+ pharmacies, and everyday medications delivered home in 90-day supplies. At every level our providers can prescribe to your pharmacy.
Therapy+ is included at the Complete level — up to 12 sessions with a licensed therapist per major life event. Mental health evaluation and medication management are included at every level.
Babies can join from six months old — add them to your household then.
Members pay $0 for office visits at both Charleston clinics.
Family+ has a 90-day minimum, then 30 days’ notice. You can give notice from day 60, so your membership ends right at day 90. There is no registration fee.
Yes. West Ashley at 2039 Savannah Highway and North Charleston at 5479 N. Rhett Avenue are both included, and members are seen within 15 minutes, 93.7% of the time.
